Trial Adoption Addendum

I, {{Adopter Full Name}}, the undersigned, do enter into this contract (this “Agreement”) with Muddy Paws Rescue, Incorporated (“MPR”) in respect of the dog known as {{Dog Name}} with microchip number {{Chip Number}} (“Dog”). I acknowledge that I am aware of, understand, and agree to the following:
1. I understand that MPR is offering the Trial Period for the Dog in an effort to help the Dog find an adoptive home, and can state in good faith that I have every intention of keeping the Dog at the close of the Trial Period barring any discoveries during the Trial Period that render the match incompatible. I agree to care for the Dog until {{Trial End Date}}, at the earliest, (with the period of time between the contract execution date and {{Trial End Date}} constituting the “Trial Period”).
2. During the Trial Period, I agree to comply with the following policies and procedures:
I will not permit the Dog to engage with other unknown dogs on leash or enter a dog park or run to socialize with unknown dogs;
I will ensure that the Dog is leashed any time the Dog is out of my home and that the Dog wears its MPR-issued martingale collar with identification tags and walking device at all times;
I will crate the Dog anytime I cannot provide active supervision; and
I will ensure that the Dog remains in my care and supervision and not that of another person, unless previous permission is granted by MPR.
3. I understand that these policies are not optional and any breach of policies will be considered a breach of this Agreement, and that the purpose of these policies is to set the Dog up for success and provide structure and consistency for the Dog. If I do choose to exercise my option to return the dog to MPR at the end of the Trial Period and I am found to have violated any of the above policies, I will not be entitled to receive a refund of my adoption fee.
4. With the sole exception being real, extreme danger to a human in my household that is directly the result of the actions of the Dog, I understand that I will be required to care for the dog until at least the close of the Trial Period, regardless of whether I have decided that the match is incompatible prior to the end of the Trial Period. I understand that a dog’s behavior can change in new environments, and that it is my responsibility to provide a structured, consistent environment for the Dog.
5. I understand that if I will be returning the Dog to MPR, it may take MPR 5 business days or longer from the time that I inform MPR of such return to identify a new foster home, during which time the Dog may be made available publicly to other potential adopters. It will be my responsibility to care for the Dog until MPR identifies a suitable foster home for the Dog and to transport the Dog to either the foster home or an MPR location, as determined by MPR.
6. If, by the last day of the Trial Period, I have not informed MPR of my intent to return the Dog to MPR, I understand that I will have fully accepted possession of, rights to, and title and responsibility for the Dog as per the above completed Adoption Agreement, as well as waived my right to a refund of the adoption fee under this Trial Adoption addendum. If, by the last day of the Trial Period, I have informed MPR of my intent to return the Dog to MPR, I am entitled to a full refund of the adoption fee unless I am determined by MPR to be in violation of this Agreement in any way.
